The Staff Pad has partnered with a reputed healthcare organization in Pomona, CA and we are looking to hire a Director of Quality Improvement provides organization-wide leadership for the organization’s quality improvement program, partnering with executive, clinical, operational, and compliance leaders to establish quality strategy and translate organizational and regulatory requirements into measurable action plans.

Operating under the leadership of the Chief Medical Officer (CMO) and Chief Operating Officer (COO), with the CMO providing direct clinical oversight of quality improvement activities and regulatory quality programs, The Director leads initiatives focused on improving patient outcomes, safety, access, equity, and care experience while overseeing quality data, clinical quality reporting, continuous improvement, Patient-Centered Medical Home recognition, and the overall performance of the Quality Improvement Department.

What You’ll Do

Key Responsibilities

Quality Strategy and Governance

• Lead the organization’s Quality Improvement and Quality Assurance Plan and annual evaluation
  
• Establish quality priorities, measures, targets, owners, and timelines; monitor progress and corrective actions
  
• Lead/support Quality Improvement Committees and workgroups and prepare leadership and Board reports, agendas, dashboards and meeting materials.
  
• Advise leadership on quality risks, trends, resources, and improvement opportunities
  
• Promote continuous improvement, accountability, patient safety, and learning
  

Department Leadership

• Direct Quality Improvement staff, including hiring, supervision, coaching, evaluations, workload, and development
  
• Develop departmental goals, staffing plans, procedures, and budgets
  
• Ensure accurate, timely, and useful quality analyses, audits, and reports
  
• Collaborate with Clinical, Operations, IT, Compliance, HR, Finance, Behavioral Health, Dental, and other departments
  

Quality Measurement and Reporting

• Oversee collection, validation, analysis, and submission of UDS, HEDIS, payer, grant, population health, and internal quality measures
  
• Develop dashboards for leadership, providers, sites, and service lines to track performance and improvement
  
• Monitor provider and care-team performance, variation, and care gaps while maintaining confidentiality
  
• Establish data-validation and governance controls and partner with Data and IT leadership
  
• Present quality findings to the Board, leadership, staff, health plans, and external reviewers
  

Performance Improvement

• Lead PDSA cycles, root-cause analyses, and other structured improvement initiatives
  
• Partner with clinical and operational leaders to improve quality, access, outcomes, patient experience, and care coordination
  
• Translate data into sustainable workflow improvements, training, decision support, audits, and accountability processes
  
• Evaluate results, identify lessons learned, and spread effective practices
  

Regulatory Readiness and Patient Safety

• Lead quality activities for Patient-Centered Medical Home recognition and renewal with Compliance
  
• Support HRSA, FTCA, health plan, accreditation, risk-management, and other external reviews
  
• Support corrective action plans, monitor completion, and verify effectiveness and sustainability
  
• Analyze patient safety events, grievances, and quality-of-care concerns while maintaining confidentiality
  
• Maintain quality plans, reports, audits, committee records, improvement projects, and supporting documentation
  

Clinical Analytics and Health Information Technology

• Direct clinical analytics and reporting to support decision-making, population health, and performance improvement
  
• Partner with IT and vendors to improve data extraction, interfaces, registries, decision support, and reporting tools
  
• Lead quality data governance, including definitions, ownership, validation, access, and change control
  
• Evaluate digital tools and automation and recommend safeguards, testing, and monitoring
  
• Recommend workflow improvements involving the EHR, patient portal, population health platforms, and other clinical systems
  

Organizational Responsibilities

• Participate in meetings related to quality and organizational performance
  
• Maintain knowledge of FQHC requirements, quality measures, health plan expectations, and improvement practices
  
• Protect confidential information and comply with HIPAA, cybersecurity, and records-management requirements
  
• Perform other related duties as assigned
  

What We Are Looking For

Education and Experience

• Bachelor’s degree in public health, healthcare administration, nursing, health sciences, informatics, or related field required; master’s preferred
  
• 5+ years of progressive healthcare quality, population health, clinical analytics, or program management experience, including 3+ years in supervisory or department leadership
  
• FQHC, community health center, or safety-net healthcare experience strongly preferred
  
• Experience leading cross-functional improvement projects and preparing quality reports for executives, boards, health plans, or regulatory agencies required
  
• CPHQ, Lean, Six Sigma, or comparable quality-improvement certification preferred
  

Knowledge and Skills

• Knowledge of quality improvement, patient safety, clinical quality measurement, health equity, and population health
  
• Knowledge of HRSA Health Center Program requirements, UDS, HEDIS, PCMH standards, and payer quality programs preferred
  
• Advanced ability to collect, validate, analyze, and present healthcare data and translate findings into improvement strategies
  
• Proficiency in Excel and PowerPoint, with experience using EHRs, population health, data visualization, or clinical reporting tools; NextGen preferred
  
• Strong leadership, project management, facilitation, communication, and presentation skills
  
• Ability to manage multiple priorities, meet deadlines, exercise sound judgment, and communicate sensitive information professionally
  
• Commitment to serving medically underserved communities and reducing healthcare disparities

Work Environment

• Primarily office and health center-based, with regular travel among sites and occasional travel for meetings or training
