WCS Healthcare Partners is working with a social services organization based in the Bronx, NY that provides support and services to individuals with intellectual and developmental disabilities (IDD), helping them live as independently and fully integrated into the community as possible.

 

The Registered Nurse (RN) will provide direct and supportive healthcare to individuals residing in the organization's residential programs. This position focuses heavily on care coordination/case management, health monitoring, documentation, and staff training/oversight to ensure individuals receive appropriate and quality care.

 

The RN will be responsible for:

• Coordinating healthcare and case management services for individuals residing in residential programs
• Monitoring and documenting individuals' health status, medical needs, and follow-up care
• Providing healthcare support to individuals with complex and medically fragile needs
• Training and providing oversight to direct care staff regarding healthcare procedures and the provision of care
• Communicating with families, healthcare providers, and other members of the care team
• Attending staff meetings and participating in company functions as needed
• Working independently while managing multiple individuals and healthcare needs

• Monday–Friday
• Full-Time

 

Required Qualifications

• Active New York State RN License
• Strong computer literacy skills are required
• Bachelor's degree in Nursing
• At least 1 year of experience providing care to the OPWDD population OR experience as a home care case manager
• Ability to work independently and manage responsibilities with minimal supervision

 

Preferred Experience

The ideal candidate will have a background that demonstrates progressive clinical responsibility and independence. Preference will be given to candidates who:

• Previously worked as an LPN and later advanced to an RN
• Have leadership, charge nurse, or supervisory experience at the LPN level
• Have Long-Term Care (LTC) experience
• Have previous OPWDD/IDD experience
• Have experience working independently in a similar residential, community-based, or healthcare setting
• Have previous Medical Assistant (MA) experience, which may also be considered

 

Transferable Experience

Candidates without direct OPWDD experience may be considered if they have at least 1 year of relevant experience working independently as an RN, including backgrounds in:

• Sub-acute care
• Home care
• Long-Term Care
• Mental health
• Other comparable clinical settings requiring independent patient management and care coordination

 

Clinical Skills

Experience caring for medically fragile or complex patients is preferred, including:

• G-tube feedings
• Foley catheter changes
• Hoyer lift usage
• Other hands-on care for individuals with complex medical needs

 

Physical & Job Requirements

Candidates must be able to manage the mental and physical demands of the position, which may include:

• Assisting with lifting or repositioning individuals for care and bed changes
• Responding quickly to individuals when necessary, including running after a patient if needed
• Climbing stairs
• Managing physically and medically demanding situations in a residential setting

 

Computer & Communication Skills

Strong computer skills are required for:

• Maintaining and updating detailed patient records
• Documenting health status, medical needs, and follow-up
• Communicating with family members, healthcare providers, and other medical staff
• Managing electronic documentation and other required systems
