Position Title: Care Navigator
Work Location: Remote; FL
Assignment Duration: 12 months (Possibility to extend or convert)
Work Schedule: Monday-Friday, 8 am - 5 pm; No OT
Work Arrangement: Remote
Position Summary:
The Care Navigator team works with low-medium acuity members in the foster care system or adopted from the foster care system. This position will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and providing community resources to address social determinants of health needs.
Key Responsibilities:
• The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.
• The position specifically is providing case management/care coordination to members who qualify for our Intellectual and Developmental Disabilities Management (IDDM) program, which may include members with a developmental diagnosis, autism or low IQ. They may be on the APD waiver or waitlist program.
• After training is concluded, monthly audits are completed with a passing score of 90% or higher.Staff are expected to complete timely outreaches (ranging from once a month to once a quarter) to guardians, parents, providers, agency staff to assess for needs and enter notes in within 1 business day.
Qualification & Experience:
• Requires a Bachelor’s degree and 2 – 4 years of related experience. Requirement is Graduate from an Accredited School of Nursing if holding clinical licensure. Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.
• 2-4 years working in community mental or in the child welfare field.

• Experience working within the child welfare field/foster care field and a desire or experience working with IDDM population. • Additional qualities to look for: Bachelor's degree in behavioural health or social work-related degree.
