Position Title: Senior LTSS Service Care Manager (RN)
Work Location: San Antonio and surrounding areas- Bear County and counties that directly touch Bear County.
Assignment Duration: 6 Months with a Possibility to extend or convert
Work Schedule: 8:00 am – 5:00 pm Monday-Friday with possibility for OT
Work Arrangement: Remote with field visits

Position Summary: We are the STAR Kids Assessment Team who completes initial assessments for the new children that come on to the STAR Kids program with The Organization. We assess for any potential unmet needs the child may have or assess for any Medicaid services in which the child could benefit from based upon their condition.

Background & Context: We work remotely from home and have multiple Teams chats in which we communicate/ask questions. Myself and my Team Lead are always available for questions and promote an open communication environment.

Key Responsibilities:

• Work schedule 8:00 am – 5:00 pm Monday-Friday.
• We completed 6 member visits per week with 2 days for documentation.
• Typically have 3 days with 2 visits per day and 2 full days for documentation.
• Member visits completed in member’s home so travel from employee’s home office to member’s home and back is required.
• We do work with the pediatric population so we are scheduled visits at 3:00pm which have a great potential (with drive home) to go past 5:00 pm.
• This is also a documentation heavy position with specific turnaround times for documentation to be submitted.
• Performance metrics are measured by documentation and training compliance as well as set company goals.

Years of experience required: More than 3 years nursing experience and 2 years case management experience Disqualifiers:   Additional qualities to look for: Home Health experience, prefer candidates with pediatric experience
