Job Title: Data Management Coordinator
Location: 2011 N Soto Street, Second floor, Los Angeles, CA 90032 , On - site 3-4 times a week, this will depend on training no guarantees
Duration: 3 months
If remote will client be providing equipment? Only a laptop
Work hours: 8am-4:30pm
Interview: HM will be interviewing the candidate via teams VIDEO. I might include another employee but not too certain at this time.

Must-Have Qualifications & Skills:
- Bilingual fluency
- Medical records experience in a healthcare setting (preferably transplant or specialty care)
- Strong EMR experience (ability to request, obtain, upload, and index records accurately)
- Excellent verbal and written communication skills (patient-facing and clinical-facing)
- Strong customer service skills, including phone and in-person patient interaction
- Ability to manage urgent requests and escalate time-sensitive clinical issues appropriately
- Analytical skills to review patient charts, physician notes, and testing documentation
- Attention to detail to ensure records are placed correctly and compliantly in the EMR
- Ability to problem-solve in real time and work independently in a fast-paced environment
- Ability to learn hospital and transplant IT systems quickly
- Experience coordinating across teams (clinical, scheduling, quality, vendors, outside facilities)
- HIPAA compliance awareness and handling of protected health information
- Transplant program experience
- Medical terminology knowledge
- Experience with patient education materials and consent validation
Job Description:
- The data management coordinator fulfills a variety of roles to provide continuity of care for patients.
- The data management coordinator is an essential component of the transplant team and works in conjunction with clinical, financial and scheduling colleagues to assure patient records are requested, obtained and placed into the EMR in a timely and efficient manner for review and documentation.
Duties include:
- oversight of patient medical records, creating patient education packets, responding to urgent records requests, setting up the patient education class and validating patient consents.
- S/he provides stellar customer service when interfacing with patients in person and over the phone, alerting clinicians when patient testing is uploaded, escalating urgent issues, and engaging outside facilities for patient medical records.
- The data management coordinator is also expected to problem-solve in real time and provide appropriate direction and support to patients and staff in regards to medical records.
- The data management coordinator will participate in team meetings and discussions related to complex cases and also advance operational throughput and process improvement to benefit patient care and record retrieval.
- S/he should possess analytic skills and be able to evaluate/analyze information, patient charts and physician notes to effectively obtain and upload patient records and convey/communicate information as needed to others.
- S/he should have excellent verbal and written skills and an ability to easily learn hospital and transplant IT systems as they relate to record retrieval.
- The data management coordinator acts as the liaison between clinical, scheduling and quality teams to assure patient records are being effectively obtained and placed in the EMR to the correct location which is compliant with transplant/hospital standards and legal requirements.
- Bilingual fluency is strongly preferred.
