Job Description

Position Title: Care Manager (RN)
Work Location: Region 5 - St. Petersburg ideally Assignment
Duration: 3 Months Contract to Hire
Work Schedule: M-F 8am – 5pm

Position Summary:
This position will be with the Enhanced Care Coordination team for Children’s Medical Services plan within our organization. This team serves medically fragile children aged through 21 years. Specifically, this team primary serves members with private duty nursing services in the home.

Key Responsibilities:

  • Day-to-day: Making calls to parents of members to collect annual assessments, monthly required update call, a monthly face-to-face visit and if visit declined a second monthly call or a virtual visit replaces it.
  • Calling DME companies and providers if a parent reports a problem.
  • Managing difficult parents at times, responses to escalations and/or complaints as needed.
  • Monthly regulatory reporting compiling updates obtained on each member of their caseload.
  • Overall expectations: Requires good communication skills, self-motivation and time management, and EXCELLENT documentation.
  • Metrics include completion of all required calls/contacts/visits or calls in place of visits, collection of assessments on time, compile monthly information regarding member status that is required by end of month.
  • Zoom calls or a phone call with the PDN agency every 6 months to review the care plan agency has provided.

Candidate Requirements
Education/Certification Required: ASN Preferred: BSN
Licensure Required: RN Preferred: N/A
Years of experience required: 2 years of pediatric experience is required per contract.
 
Disqualifiers: Poor documentation, poor time management skills, lack of technology experience, not enough pediatric experience as an RN in the clinical or home care setting. 2 years continuous pediatrics is key to truly understanding this population.
 
Additional qualities to look for: Nurses that have worked in ICU for pediatrics/NICU or pediatric home health do well in this position. Prior case management or managed care experience is helpful but not required. At least a basic understanding of Microsoft programming (Outlook, Word, Excel) is essential.

1 2 years of pediatric experience required.
2 Field visits required.
3 Computer/tech savvy.
