Working Job Title: Patient Coordinator/ Referral Services Specialist
Duration of Assignment: 13 weeks with the possibility to extend or convert to FTE
Work Location: Alhambra, CA 91803 (open for remote as well)
Work Hours: 8:00 AM – 5:00 PM
Interview Process: Two-step interview process (Video/Teams)
Dress Code: Business Casual
Remote Work and Onsite Expectations:
- Remote work is acceptable, with a preference for local candidates ONLY as they will need to pick up equipment on their first day.
- Occasional onsite requirements may include HR orientation, equipment pickup, pre-employment health appointments, annual team events, go-lives, or other manager-assigned instances.

Must-Have Qualifications:
- 2–3 years of experience in scheduling, case management, or utilization review
- Bilingual in English and Spanish
- High school diploma or equivalent
- *****Experience with document review and identifying missing documentation
- 3 years of healthcare experience
- Strong organizational and time-management skills
- Demonstrated excellent customer service skills
- Excellent verbal and written communication skills
- Proficiency in using computer systems and required equipment relevant to the role
Preferred Qualifications:
- Graduation from an accredited Medical Assistant or LVN program
- 1 year of experience in scheduling, case management, or utilization review
Preferred Certifications:
- Certified Medical Assistant (CMA) or
- LVN with a valid California license
Position Summary:
In collaboration with an interdisciplinary team, the Referral Services Specialist provides clinical and financial care coordination services by evaluating care options and services needed to meet individual healthcare needs. This role promotes cost-effective, high-quality outcomes and serves as a consultant to the healthcare team for managing specific patient populations.
The Referral Services Specialist works closely with patients, families, physicians, and care teams to:
- Assess individual and family needs
- Facilitate the development of interdisciplinary care plans
- Coordinate care across multiple service sites
- Evaluate the effectiveness of services
- Negotiate resources to support optimal outcomes
This role also directs patient admissions and manages the intake process, collaborating with physicians and physician extenders to review medical admitting information, assess medical necessity, and ensure appropriate and complete documentation.
