The Role:
As the Director of Care Management, you will spearhead the development and execution of a cutting-edge Care Management system. Your role is pivotal in ensuring patients receive timely, appropriate care, identifying treatment delays, and optimizing resource utilization. Your leadership will guarantee seamless patient discharge planning and continuity of care. You will manage diverse departments across the continuum of care, fostering integrated services while maintaining fiscal responsibility.
What You’ll Do:
- Improving Performance: Cultivate a culture focused on excellence, ensuring organizational and department policies are current and adhered to, and champion quality improvement processes.
- Communication: Foster open, honest, and respectful exchanges, effectively communicate strategic initiatives, and encourage collaborative decision-making.
- Fiscal Stewardship: Take charge of the fiscal management of departments, formulate and defend annual budgets, and implement cost-effective strategies.
- Competency: Develop a nurturing work environment promoting growth and accountability, assess staff development needs, and support employee growth.
- Planning: Drive strategic and operational plans aligning with the mission, vision, and values, anticipate changes, and promote integration with community resources.
What You’ll Bring:
- Educational Background: Bachelor’s Degree in Nursing required, Master’s Degree in Nursing or related field preferred.
- Experience: 10+ years in acute healthcare care management, 8 years in leadership, knowledge of payer billing, regulatory requirements, and clinical assessments.
- Skills: Leadership, strategic planning, communication, problem-solving, critical thinking, and interpersonal skills.
- Knowledge: Familiarity with healthcare regulations, resource availability, family development, statistical data analysis, and care management techniques.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
