Position Title: Senior LTSS Service Care Manager (RN)
Work Location: Laredo, TX
Assignment Duration: 6 Months (possibility to extend/convert)
Work Arrangement: Hybrid\
Schedule: M-F/8-5 with possibility of OT when warranted

Position Summary:

This position is for the Laredo area to complete assessments in member’s homes as well as completing documentation as per State and Company guidelines to ensure compliance.

Key Responsibilities:

• M-F visits 3 days out of the week, with OT possible with manager approval, documentation days 2 days out of week, traveling to members homes to complete assessments/documentation
  Face to Face assessments completed 3 days out of the week with 2 documentation days, completing tasks, submitting all forms/assessments within turnaround times, completing outreaches
