Position Title: LTSS Care Navigator
Work Location: Corpus Christi
Assignment Duration: 6 months with potential of extension

Position Summary:
LVN needed to assist with follow up calls to members, MD offices, DME agencies, Provider agencies, etc.

Key Responsibilities:

• LVN needed to assist with follow up calls to members, MD offices, DME agencies, Provider agencies, etc.

• LVN will need to work on any assigned tasks within expected turn-around time.

• LVN will need to track productivity and turn in spreadsheet each week.

• LVN may need to perform field visits in Corpus Christi and surrounding towns, if needed to assist.

• LVN will need to fill out required forms and document assessments within turn-around time.

• LVN must have good computer skills and be able to work independently in home office, being mindful of privacy to avoid any HIPAA breech.

• LVN must have stable and dependable Wi-Fi services.

• LVN must have good verbal and written communication skills.

Qualification & Experience:

• LVN

• Home health experience

• MD office experience

Preferred Skills/Experience:

• Good verbal and written communication

• Comprehension of Microsoft software (excel; word; etc.)

• Quick learner

Education Requirement:

• LVN certification in BON
