Position Title: Senior Care Navigator

Work Location: Florida (Tampa, Central FL, and Miami)
Assignment Duration: 3 Months
Work Schedule: 8am-5pm ET, no OT
Work Arrangement: Remote

Key Responsibilities:

• Intake assessments on CMS members (telephonic case management)
• Set tasking of assigned intake caseload with membership between 150 – 200 members
• Virtual with a couple of in person meetings per year
• Remote/Autonomous high tech work environment with assessments and telephone contact of members
• Predominantly tier 4

Education/Certification	Required: As long as have LPN- no EDU minimum		Preferred: None
Licensure	Required: LPN active in Florida		Preferred: None
