Job Description

Position Title: Quality Outreach Coordinator
Work Location: Remote
Work Duration: 8 Months (Possibility to extend or convert)


Summary:
Performs duties to assist in activities related to closing open care gaps for high-risk members.

Key Responsibilities:

  • Interacts with high-risk members and/or providers to improve healthcare outcomes and close care gaps such as increased preventive care compliance
  • Provides support for quality initiatives including outreach to members who are noncompliant in the designated outreach services
  • Conducts seasonal call campaigns, in coordination with local market efforts
  • Enters data assessments and authorizations into the system and maintain database as directed
  • Maintains integrity of Personal Health Information
  • Performs other duties as assigned
  • Complies with all policies and standards Qualification & Experience: High school diploma or equivalent. 2 years managed care setting, medical office or facility setting with demonstration of medical administration duties. Knowledge of medical terminology.
