Title: Resource Mother I
Duration: 12 Months - Possible extension/conversion
Location: Remote- GA field-based | 1 person in Central region (Macon or closest proximity to Macon) 2nd in SW region (Albany or Valdosta)
Schedule - Monday – Friday 8:30 am-5:30 pm EST

Summary:
Responsible for outreach, support, mentoring, education, community linkages etc. of women who have delivered Very Low Birth Weight babies and are participating in the Planning for Healthy Babies program. The RM is also responsible for educating and supporting members throughout their 12-month postpartum period. Resource Mothers play a significant role in improving the Plans postpartum visit HEDIS measure as well as carrying out the contractual requirements of the Planning for Health Babies Program
 
Responsibilities
Provide education to P4HB members on plan services and benefits to encourage healthy behaviors, including healthy eating choices and smoking cessation.
Complete intakes based interviews with P4HB participants and supporting to identify the needs of the members.
Identify and document member needs and provide to clinical management to develop plan of care.
Coordinate and implement the delivery of member care and social services within the community.
Assist participants in locating and utilizing community resources including legal, medical, financial assistance, and other referral services.
Coordinate services to assist participants with attending primary care medical appointments.
Serve as the liaison between participants and family members, significant others, nurses, physicians, and organizational components to facilitate communication, linkage and continuity of service.
Document status of appointments, enrollment and participation in planned services and benefits.

Outreach metrics: Ensuring this is completed in a timely manner: SOP’s that are in line with the processes as well as the metrics that are aligned with the processes.
 

Candidate Requirements
Education/Certification	Required: High school diploma or equivalent required. 2+ years of experience working with community agency providing services encompassing the basic competency areas of health care, managed care, member outreach (community resources, communication skills, individual and family advocacy, health education, services skills and responsibilities).			Preferred: Associate degree in related field preferred.
Licensure	Required: N/A- just driver’s license			Preferred:
Years of experience required: 2+ years’ experience (reference Education req info above)   Disqualifiers: Outside of contract work; no job-hopping and short tenure positions (MUST NOTE IF CONTRACT POSITIONS ON RESUME)   Additional qualities to look for: Home visits experience
