Position Title: Resource Specialist
Work Location: Region A, Florida (Counties: Santa Rosa, Escambia, Okaloosa, Walton, Holmes, Washington, Bay, Jackson, Calhoun, Gulf, Liberty, Franklin, Wakulla, Leon, Gadsden, Jefferson, Taylor, Madison, Dixie)
Assignment Duration: 3 Months (Potential to Extend)
Work Schedule: 8am-5pm EST, Monday thru Friday (Weekend work may be needed during Hurricane Season)
Work Arrangement: Hybrid / Field-based (Work from home + required field visits)

Position Summary:
Individual will work from their home and will also be required to go into the field. Field visits could include home visits, visiting members in group home/hospital setting, or attending community/agency meetings.

Background & Context:
This position will be field based and will provide care management to assist with coordination of care for members in the designated Region A.

Key Responsibilities:
• Provide care management to assist with coordination of care, including assessing for health care needs.
• Refer to health plan providers and provide education on benefits/expanded benefits.
• Provide community resources to address social determinants of health needs.
• Manage a caseload of 100 members.
• Document in an electronic health record including outreach notes, complete assessments, and complete a care plan on each member.
• Comply with care management policies/procedures including outreach and documentation expectations.

Qualification & Experience:
• Bachelor’s Degree.
• Pediatric case management experience.
• Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.
• Microsoft Office - Outlook, Word, and Excel.

Working Conditions & Physical Demands (If Applicable):
Field visits required (home visits, group home/hospital settings, community/agency meetings). Weekend work possible during Hurricane Season for disaster outreach.
