Job Description

Title: Nurse Practitioner (NP)/Nurse Case Manager
Duration: 12 Months
Location: NY, New York 10001
Onsite
First 6 months- 5 days onsite, then 4 days/week onsite and 1 day remote
8:30 AM to 4:30 PM (Monday – Friday)


Descrition:
Provide clinical services to employees within regulated scope of practice and disability case management.

Experience/Education:
• MS/NP in nursing degree and current NP license in New York State
• Master’s Degree in Advanced Practice Registered Nursing
• Current APRN Board Certification
• Minimum of 3-5 years' experience as an occupational nursing professional
• Minimum experience of 1-3 years in case management, utilization review and/or quality assurance
• Experience in episodic care, wellness care and counseling of adults
• Proficiency in use of electronic medical record keeping systems desirable
• BLS certification required
• Optional: Case Management and COHN-S certifications

Major Responsibilities:
• Assist in coordination of clinical operations, including medical surveillance, OSHA recordkeeping, facility operations and patient care activities such as episodic or emergency visits, first aid, and other activities as needed.
• Provide occupational and non-occupational injury response, evaluation, and treatment care
• Provides preventive medicine and wellness services/counseling; performs physical examinations, supports site health promotion and disease prevention programs
• Documents patient data in electronic medical database(s), as per standard protocols
• Manage and process Medical Leave of Absences (Short Term Disability, FMLA, ADA) by tracking claims and communicate case status to employees, mangers, human resources, legal, and healthcare providers
• Facilitate and be a resource to employees in a smooth transitional (modified) or full return to work assignments

Technical Competency Description:
• Health Insurance Portability and Accountability Act (HIPAA)
• Americans with Disability Act (ADA)
• Family Medical Leave Act (FMLA)
• Workers’ Compensation
• OSHA
• OSHA Blood-borne pathogen/Universal precautions

Clinical Skills:
• Phlebotomy experience required, perform EKGs, etc.
• Administer vaccinations
