LTSS Service Care Manager

Title: LTSS Service Care Manager
Duration: 4 Months
Location: Remote – California, PST
M-F 8am to 5pm, PST | Possible OT

Position Purpose:
Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs
 

Background & Context:
Client will discontinue the ALF Transition community support service. The discontinuation will require that we support the transition of our vulnerable members from ALF setting to alternate setting, including home, SNF/LTC, RCFE/ARF. Team members will carry a caseload of 300+ members they will support through these transitions.

1. Call ALWs to follow up on member transitions and member needs
2. Speak to SNF/LTC providers, Members and their families about transitions

 
Performance metrics – we will track all transitions.
 

Candidate Requirements
Education/Certification	Required: Bachelors Degree			Preferred: Social Services/Social Worker
Licensure	Required: None			Preferred: None
Years of experience required: 3+ years of SNF, ALF, Social Worker experience, MS Office applications   Disqualifiers: scared to speak to Members, families, providers   Additional qualities to look for: ability to multi task, document conversations, manage data
