LTSS Service Care Coordinator

Position Title: LTSS Service Care Coordinator
Work Location: Remote – IL, specifically Candidate must reside within Madison County (Alton, Godfrey, East Alton)
Assignment Duration: 6 Months Contract to Hire
Work Schedule: Mon - Fri 8am CT– 4:30pm CT with 30 minute lunch. No OT
Work Arrangement: Remote (home-based with required field visits to members’ homes)

Position Summary: LTSS Case Manager, specifically managing the following HCBS waivers: Physical Disabilities and Aging. The individual works from home and is required to have their own car and a valid driver’s license to visit members in their home to conduct health risk assessments and create care plans and service plans.

Background & Context: Backfill for FTE. This individual will be within the following geographic team. The individual works from home and is required to have their own car and a valid drivers license to visit member’s in their home to conduct health risk assessments, create care plans and service plans. Additional hiring needs have been submitted.

Key Responsibilities:

• The individual works from home and is required to have their own car and a valid driver’s license to visit members in their home to conduct health risk assessments, create care plans and service plans
• They are required to have a home office space to complete documentation and phone calls necessary to coordinate care and conduct follow up assistance
• Experience coordinating services among members, caregivers, providers, and community partners, completing assessments and developing person-centered care plans
• Knowledge of HCBS waiver services, Medicaid programs, and community-based resources
• Strong documentation skills with the ability to maintain accurate, compliant member records
• Complete all documentation within 2 business days from the date of contact, follow all processes, policies and procedures, complete internal audit remediation within 14 days of receiving audit notification and meet all deadlines provided

Education/Certification	Required: Required: Requires a Bachelor's degree and 1 year of related experience.   Physically Disabled/Elderly Candidate must meet one of the 3 following criteria: 1. RN licensed in Illinois. 2. Bachelor or Master’s Degree prepared in human services related field. Bachelor’s degree in Human Services related field defined as: Child, Family and Community Services, Early Child Development, Guidance and Counseling, Home Economics- Child and Family Services, Human Development Counseling, Human Service Administration, Human Services, Master of Divinity, Pastoral Care, Pastoral Counseling, Psychiatric Nursing, Psychiatry, Psychology, Public Administration, Rehabilitation Counseling, Social Science, Social Services/Social Work or Sociology. 3. LPN with one (1) year experience in conducting comprehensive assessments and provision of formal service for the elderly			Preferred: NA
Licensure	Required: none required			Preferred: NA
Years of experience required: At least 1 year   Disqualifiers: No experience as a field based LTSS care coordinator managing HCBS waivers   Additional qualities to look for: Highly organized
