Position: Licensed Certified Addiction Counselor (LCAC) Location: Portsmouth VA 23701 Duration: 13 Weeks Shift: DAYS - 8HR: Days with weekends if needed; holidays as assigned Description: Must be able to float between Safe Harbor facilities. 2 miles or less apart Participates in client treatment planning and continuum of care. To facilitate clinical groups and psychosocial assessments and formulate treatment plans. There may be some administrative tasks as well. Conducts group and/or family sessions, under supervision, as scheduled and documents as required. Co-facilitates assigned group or family sessions and assists with modifying problem behaviors. Responsible for initial assessments, as well as follow up assessments for clients. Prepares individual treatment plan for each assigned client. Ensures all documentation regarding client care, treatment and incidents is completed timely and in a clear, concise manner. Acts as a liaison between referral sources and clients. Provides crisis intervention to clients, as needed. Plan for aftercare for assigned clients. Acts as a liaison with insurance carriers, as requested. Provides case management duties for clients, ensuring individualized quality care. Evaluates the need of the client and determines if referrals to other programs or facilities is needed. Provides client and family education relative to substance use disorder/psychiatric problems. Conducts educational workshops both within the Facility and in the community as required. Serves as a liaison with self-help groups and EAP professionals. Conducts ongoing assessments of chemical dependence services required by the Facility treatment programs. Assists in the training of new clinical staff and interns, providing direction and clinical guidance. Coordinates the activities of other clinical and direct client care staff, providing direction and clinical guidance. Acts as LEAD for the program/unit. Additional Responsibilities: Complies with organizational policies, procedures, performance improvement initiatives and maintains regulatory and industry standards. Maintains regular and reliable attendance. Accepts responsibility for professional growth and development of self and attends all mandatory facility in-services and staff development activities as scheduled. Maintains professionalism and confidentiality of facility employees and clients. Performs other duties as assigned.
