Clinical Pharmacist

Title: Clinical Pharmacist 
Duration: 4+ Months
Location: Remote - SC

Shift - 8am-5pm, potential night/weekend

Description:

YEP/HEDIS – Working excel reports for member/provider outreach around medication adherence gap closure for multiple Quality HEDIS measures. Some will have a focus around SPC/SPD and others could be focus around Behavioral health measures. Performance expectations would include completing outreaches daily to members/providers/pharmacy for gap closures. Metrics would depend on volume and which HEDIS measure is being worked
