Job Description

Title: Care Manager (RN)
Duration: 03 Months (Contract to Hire) 
Location: Member Facing -
 Hillsborough –– Polk/ Hardee, Highlands
(Spanish Language- Required)

Note: Individual will work from their home and will also be required to go into the field.
Responsibilities:
  • This person will have metrics of calls that will need to be completed.
  • Phone calls.
  • Coordinate care (finding providers)
  • Assisting in authorization
  • Creating assessments and care plans
  • Ensuring that the children’s needs are met (having issues at school, having a home health aide to assist)
  • Weighted 200 (depends on their acuity)
Internal/External Groups with which the Candidate will interface:  
Required Skills/Experience:   Preferred Skills/ Experience:  
1. 2 – 4 years of related experience 1.  
2. 2 years of peds experience within case management 2.  
  Comfortability to complete high volume of calls while being able their caseload    
3. Comfortability with face-to-face visits 3.  
Education Requirement: Requires a Degree from an Accredited School of Nursing or a bachelor’s degree in nursing and 2 – 4 years of related experience. Education Preferred:

Required Certifications: RN - Registered Nurse - State Licensure and/or Compact State Licensure required Evaluates the needs of the member, barriers to accessing the appropriate care, social determinants of health needs, focusing on what the member identifies as priority and recommends and/or facilitates the plan for the best outcome Required Testing: n/a
