Position Title: Care Manager (RN)
Work Location: Remote, FL (anywhere in FL)
Assignment Duration: 3 months
Work Schedule: 8 am - 5 pm
Work Arrangement: Remote, FL (anywhere in FL)
Position Summary:
• Enhanced Discharge Planning Team (EDCP), who make outbound calls to Medicaid health plan members who are discharged from the hospital to prevent re-admissions.
Background & Context:
• EDCP team assesses immediate needs/barriers, complete transition of care assessment by day 10 post discharge day and attempt to remove barriers and meet needs and follow up post discharge appt with a medical or BH provider.
• Work from home 8 am-5pm
• This position is to fill in for an FTE who is on Military Leave until 6/2029
Key Responsibilities:
• Fast paced, busy day, working on your own from home with phone, email or Team IM support from supervisor, Lead and team members
• Clock in and be ready to start outreaches at 8 am.
• Receive and review new assignments daily by 8:30 am
• All newly assigned cases (10 daily) must be contacted same day as assigned and make follow up calls to members providers and venders
Qualification & Experience:
• Required Associate or Baccalaureate degree in nursing
• Preferred: Baccalaureate degree in nursing
• RN (compact is okay)
• Preferred: RN
• Must haves: 2-3 years Care Management experience with all age groups
• Nice to haves: Discharge planning experience; Care Management Certification
• Disqualifiers: Cannot answer questions regarding your past work experience during interview.
• Performance indicators: Audit scores 90% or above monthly; 1:1 meetings with Supervisor
