Care Manager

Title: Care Manager
Duration: 03 Months (Contract to Hire) 
Location: Member Facing - Santa Rosa, Okaloosa, Walton Counties
 

Responsibilities: Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families. This person will be placing calls to members / providers. Home visits depending on member needs. Assessments / charting Phone calls and computer-based work when they are not meeting with members. Metric based work. Required to make and complete certain call metrics with the members. Each member must have a completed call or at least (3) attempts (monthly / quarterly) Charting standards
