Job Title: Patient Care Coordinator
Department: Pharmacy Operations
Duration: 4+ months
Location: 13501 Park Vista Blvd. Fort Worth TX USA 76177 (The candidate should live within a reasonable driving distance)
Work Schedule: Monday to Friday 10 to 7pm
Note: The candidate should be able to work the following schedule: Monday to Friday 10 to 7pm, Training is Monday to Friday 9 to 6pm; Occasional Saturdays 9 to 6pm
Job Responsibilities:
Purpose
The Patient Care Coordinator is responsible for performing intake and Rx Setup (facilitating the
processing of prescriptions) activities, and functions in accordance with company policies and procedures to assist in the flow of dispensing drugs to the patient.
Essential Duties and Responsibilities
? Review contents of referral to ensure complete and accurate information has been received
? Create a patient profile from a scanned or faxed image by entering correct patient and provider information to ensure all other groups can successfully serve the patient
? Conduct initial check of prescription to ensure clinical accuracy for processing
? Communicate with providers regarding referral status, prescription clarification, missing information, or patient concerns so that all information is collected and accurate before it reaches other teams for furthering processing
? Updating concise progress notes to ensure other roles within the workflow can quickly assess the status of the referral
? Enter new provider information (i.e., National Provider Identifier) accurately into the system so that the Business Intelligence group can complete their reporting
? Perform other duties as assigned
Qualifications/Requirements
? High School Diploma required
? Healthcare experience/2+ years pharmacy experience preferred
? Current CPhT (Certified Pharmacy Technician) certification required; must be current, without restrictions
? Prior experience working in a Pharmacy setting required
? Effective communicator
Describe the day-to-day activities/responsibilities of this position:
Review reports for errors
Should be comfortable working independently with minimal guidance from management.

Candidate should have the following non-negotiable skills (Please ensure it is clearly shown on the resume before submittal):
• Need to be registered with the Texas Board of Pharmacy and CPhT in good standing
Need good data entry skills | Good attention to detail | Need to be able to come on site daily on time and good attendance
