Position Title: Care Navigator
Work Location: Remote-FL
Assignment Duration:12 months with possibility of extension
Work Arrangement: Monday-Friday, 8a-5pm; No OT

Job Description:

• The Care Navigator team works with low-medium acuity members in the foster care system or adopted from the foster care system. This position will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.
• Backfill for FTE
• We are a fully remote WFH team with no field work required. Occasionally there will be volunteer opportunities within the community. This position would sit on a team of 14 other Care Navigators and we have 60 total Care Navigators on our team (four teams of 15 each)

Candidate Requirements
Education/Certification	Required: Requires a Bachelor’s degree and 2 – 4 years of related experience. Requirement is Graduate from an Accredited School of Nursing if holding clinical licensure.   Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position. Years of experience required: 2-4 years working in community mental or in the child welfare field.   Disqualifiers:   Additional qualities to look for: Bachelors degree in behavioral health or social work related degree
