WCS Healthcare is currently seeking a Clinical Assessment Manager (RN UAS) to work with a large social services agency serving Brooklyn, the Bronx, and Manhattan, NY.
Job Responsibilities
Conduct in-home and virtual UAS-NY assessments for prospective and enrolled PACE participants to determine eligibility and ongoing care needs.
Collaborate with intake, enrollment, care delivery, and interdisciplinary teams to support seamless participant onboarding and continuity of care.
Review and update assessments, diagnoses, medications, and clinical documentation to ensure accuracy and compliance within EMR and UAS systems.
Develop proposed service plans and complete PCAT evaluations to assess ADL/IADL support needs and recommend appropriate services.
Educate participants and referral sources on PACE program benefits, services, and care delivery models.
Escalate clinical concerns, significant changes in condition, and assessment issues to quality review and care delivery teams for timely intervention.
Maintain timely and accurate documentation of assessments, service plans, and participant interactions in accordance with required deadlines.
Ensure compliance with CMS, PACE, and organizational policies, procedures, and documentation standards.
Support quality improvement initiatives, operational process enhancements, and performance metric reviews to improve participant outcomes and workflow efficiency.
Travel throughout assigned boroughs, maintain a daily assessment schedule, and serve as a clinical resource and active member of the interdisciplinary team (IDT).
Requirements
Active New York State RN License.
BSN required.
3–5 years of experience in MLTC, LTHHCP, MAP, PACE, or other managed care programs.
Minimum 5 years of experience in care/case management, disease management, population health management, utilization review, quality assurance, or discharge planning, preferably within a managed care, home care, or community health organization.
Minimum 1 year of experience working with a frail or elderly population. Candidates with less than one year of experience may be considered if they meet all other requirements and complete appropriate PACE-provided training upon hire.
Proficiency in utilizing and analyzing UAS-NY and Lenavi.
Experience with Customer Relationship Management (CRM) platforms and clinical management systems.
Strong clinical assessment, documentation, communication, and organizational skills.
