Position Title: LTSS Service Care Coordinator
Work Location: Remote – IL (Team 15 Zipcodes 60601,60602, 60603, 60604, 60605, 60606, 60607, 60608, 60610, 60611, 60612, 60613 60614, 60618,60622, 60623, 60624, 60641, 60642, 60647,60654, 60657, 60661)
Assignment Duration: 6 months (Contract to hire)
Schedule: Mon – Fri
8 am CT – 430pm CT with a 30-minute lunch

Work Arrangement: The individual works from home and is required to have their own car and a valid driver’s license to visit members in their homes to conduct health risk assessments, create care plans, and service plans

Position Summary:

- LTSS Case Manager, specifically managing the following HCBS waivers: Physical Disabilities and Aging

Key Responsibilities:

The individual works from home and is required to have their own car and a valid driver’s license to visit members in their home to conduct health risk assessments, create care plans and service plans They are required to have a home office space to complete documentation and phone calls necessary to coordinate care and conduct follow-up assistance. Performance and expectations include specific production requirements (a minimum monthly contact requirement and caseload compliance to meet contractual deadlines) as well as passing monthly audits with a score of 90% or above. In addition, must complete all documentation within 2 business days from the contact, follow all processes, policies and procedures, complete internal audit remediation within 14 days of receiving audit notification and meet all deadlines provided. Unauthorized overtime is not approved.

 

Candidate Requirements
Required: Requires a Bachelor's Degree and 1 year of related experience.   Physically Disabled/Elderly Candidate must meet one of the 3 following criteria: 1. RN licensed in Illinois. 2. Bachelor or Master’s Degree prepared in human services related field. Bachelor’s Degree in Human Services related field defined as: Child, Family and Community Services, Early Child Development, Guidance and Counseling, Home Economics- Child and Family Services, Human Development Counseling, Human Service Administration, Human Services, Master of Divinity, Pastoral Care, Pastoral Counseling, Psychiatric Nursing, Psychiatry, Psychology, Public Administration, Rehabilitation Counseling, Social Science, Social Services/Social Work or Sociology. 3. LPN with one (1) year experience in conducting comprehensive assessments and provision of formal service for the elderly			Preferred: N/A
