Position Title: Care Manager (RN)
Work Location: Florida-Remote
Assignment Duration: 3 Months (possibility to extend or convert)

Position Summary:
Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.
Background & Context:
Case Management-Medicaid. Backfill. Very Engaged Team environment, work is fast paced.
Key Responsibilities:
• Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families
• There are monthly metrics that all CM’s are required to meet including but not limited to expected cases loads
• What makes this role unique is that it is a remote position that deals directly with members
