Title: Care Manager 
Location: Remote-FL
Duration: 3 months potential to extend or convert
Shift: M-F 8-5 EST / No OT approved

Responsibilities:
Day to day will include maintaining an active caseload of about 73 Behavioral Health Complex members at a time.
Some tasks associated with this may look like -

1. Outreaching member, primary care physician and outpatient providers on a biweekly basis
2. Initiating appropriate assessment using our internal EMR system
3. Creating a Care plan tailored to the member’s needs.
4. Updating Medications, diagnosis, and allergies in our EMR system on a minimum of a monthly basis.
5. Reviewing care gaps and offer education and resources in efforts to close those care gaps.
6. Connecting member to providers and resources in the community.
7. Attending scheduled community staffings and treatment teams as well as partnering with community agencies.
8. Staff complex members as needed with internal medical directors and clinical staff.

Candidate Requirements
Education/Certification	Required: Requires a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2 – 4 years of related experience.			Preferred:
Licensure	Required: Must have an active Florida Behavioral Health License (LMHC< LCSW< LMFT)			Preferred:
Years of experience required: N/A   Disqualifiers: Not residing in Florida or not holding a valid FL BH license   Additional qualities to look for:
