Position Title: Care Manager (RN)

Work Location: Remote Florida

Assignment Duration: 3 Months Contract to Hire

Work Schedule: 40 hours per week 8A-5P. No overtime.

Work Arrangement: Remote

Position Summary: This role is a telephonic case management position and requires very little face-to-face contact with members. Due to this, the candidate will need to think outside of the box to ensure that our membership receives the care that is ordered.

Background & Context: The Enhanced Care Coordination (ECC) program is reportable to AHCA on a monthly basis due to contractual obligations. There are guided parameters and very structured. Backfill. Dealing with Medically complex pediatric members can be very time consuming and requires the ability to multitask. The CM candidate will need to be organized, self-motivated, possess excellent communication and time management skills, customer service centered, MDT Approach both internal and external customers. There are no additional upcoming hiring needs at this time.

Key Responsibilities:

• All medical fragile members are to be outreached per contractual and program guidelines.

Qualification & Experience:

Candidate Requirements
Education/Certification	Required: Nursing			Preferred: Certified Case Manager
Licensure	Required: Unencumbered FL Registered Nurse			Preferred:
Years of experience required: Have Case management and at least 2 years of pediatric experience.   Disqualifiers: No documented pediatric experience   Additional qualities to look for: Previous Case Management experience
