Job Description
Position Title: Care Manager
Assignment Duration: 12 Months (potential to extend or convert)
Work Arrangement: 100% Remote must live in Florida, Time Zone: Eastern/Central
Schedule: Monday – Friday 8a to 5p
Responsibilities:
This team manages behavioral health foster care children who are members on our plan with high acuity who have had more than one inpatient admission and who are in higher levels of care/treatment. The caseload expectation is 100 members, with at least half that are high acuity members
Contacting members/caregivers, legal guardians, providers to assess the needs of our members to ensure services are in place. Attending member staffing meetings to discuss members’ treatment progress and needs. Documentation of assessments, notes of activities on behalf of the members, and staffing meetings
Ensure members/caregivers are contacted as required, pass file audits with 90% or higher, ensure documentation is completed within 24 business hours.
These members are in the Florida Child Welfare system and being trauma informed is crucial. There is communication with the members’ caregivers and legal guardians. Members could be receiving treatment in residential facilities in and out of state.
Position is offered by a no fee agency.

| Years of experience required: 2 years. Preferred foster care and adoption experience, pediatric care management (For Sunshine Health (FL) Only: Employees supporting Florida's Children’s Medical Services must hold a Master’s degree in social work or related field and have one year of related professional pediatric care experience. May require up to 80% local travel required) Disqualifiers: Must be able to successfully pass a background check and must have a Florida license (LMHC, LCSW, LMFT). Additional qualities to look for: Passion for working with children in the Child Welfare system. | ||||
