Position Title: Senior LTSS Care Manager
Work Location: San Antonio, TX
Assignment Duration: 6 months
Work Schedule: 8:00 am – 5:00 pm Monday-Friday
Work Arrangement: Field-Based

Position Summary:

We completed 6 member visits per week with 2 days for documentation. Typically have 3 days with 2 visits per day and 2 full days for documentation.

Member visits completed in member’s home so travel from employee’s home office to member’s home and back is required.

We do work with the pediatric population so we are scheduled visits at 3:00pm which have a great potential (with drive home) to go past 5:00 pm.

This is also a documentation heavy position with specific turnaround times for documentation to be submitted.

Performance metrics are measured by documentation and training compliance as well as set company goals.

Candidate Requirements
Education/Certification	Required: RN ADN, BSN or MSN			Preferred:
Licensure	Required: Registered Nurse			Preferred:
Years of experience required: More than 3 years nursing experience and 2 years case management experience Disqualifiers:   Additional qualities to look for: Home Health experience, prefer candidates with pediatric experience
