Job Description
Position Title: Senior LTSS Service Care Manager (RN)
Work Location: Texas- Travis County
Assignment Duration: 6 Months with possible extension/conversion
Shift Schedule - Texas- Travis County
Work Arrangement: Hybrid, 2 days remote, 3 days member facing
Job Requirements:
To assist members/families with coordination and management of healthcare needs.

SC will have 3 days of visits and 2 days of documentation weekly
• SC will have a caseload and work assigned tasks (work items)
• SC will complete STAR Kids Screening and Assessment Instrument, Individual Service Plan
• SC will complete required service forms for each assessment
• SC will conduct follow up calls and complete contractual telephonic outreaches
This role is essential in ensuring members receive personalized, coordinated care that meets their unique needs. Through advocacy, education, and compassionate guidance, members and families are empowered to navigate the healthcare system with confidence, improving both their experience and health outcomes

| Licensure | Required: ADN | Preferred: BSN | ||
| Years of experience required: 4-6 years of related experience in pediatrics and home health Additional qualities to look for: Case Management Bilingual English/Spanish a plus NICU experience | ||||
| 1 | RN State Licensure and/or Compact State Licensure | ||
| 2 | Graduate from an Accredited School of Nursing | |||
| 3 | Strong organizational and time management skills | |||
