Pharmacy Technician at Kadlec Regional Medical Center

Performs functions associated with reconciliation of outpatient prescriptions (prior to admission medication list review) and other hospital pharmacy processing which are permitted by state regulations under the supervision of a licensed pharmacist. Performs routine functions assisting the pharmacist in collecting and verifying patient medication histories.

Assists the pharmacist by performing the following medication history activities, with the goal of compiling an accurate and complete medication list prior to hospital admission:

◦ Collection and clarification of the home medication list through patient or caregiver interview, information gathered from electronic sources, community pharmacies and primary care provider.

◦ Verification of allergy information.

◦ Entry of the home medication list into the electronic health record (EHR), as well as updating the patient’s preferred community pharmacy information in the EHR.

◦ Documentation in the EHR and communication of relevant information associated with home medications in the EHR and/or verbally to the pharmacist.

◦ Communicating concisely and effectively with patients, families, pharmacists, and other health care providers related to patient home medication histories.

◦ Responds to patient and provider inquiries.

◦ Performs in person and telephone reception duties in a professional manner consistent with the core values of the organization.

◦ Triages questions and consults appropriately to the Pharmacist.

Demonstrates age specific patient care considerations for the population served.

◦ Demonstrates application of knowledge, skills, and behaviors in the physical, motor/sensory, cognitive, psychosocial, patient and family education areas based on PRMCE age specific care competency assessment tool.

• Full time, Variable Shift, 40 working hours weekly

Required Qualifications:

• Upon hire: Washington Pharmacy Technician License

Preferred Qualifications:

• Upon hire: National certification
• Hospital setting Pharmacy Technician.
