Position Title: Senior Care Navigator
Work Location: Remote – Florida (Tampa, Central FL, and Miami)
Assignment Duration: 3 months
Work Schedule: 8am-5pm ET, no OT
Work Arrangement: Remote

Key Responsibilities:
• Intake assessments on CMS members (telephonic case management)
• Set tasking of assigned intake caseload with membership between 150 – 200 members
• Virtual with a couple of in person meetings per year
• Remote/Autonomous high tech work environment with assessments and telephone contact of members
• Predominantly tier 4

Qualification & Experience:
• Required: As long as have LPN- no EDU minimum
• Preferred: None
• Required: LPN active in Florida
• Preferred: None
• Must haves: Pediatrics – Minimum of 2 years
• Nice to haves: Bilingual preferred/highly encouraged, Experience with electronic medical records, Basic Excel skills
• Disqualifiers: Non licensure
• Performance indicators: Caseload compliance
• Top 3 must-have hard skills (stack-ranked):
1 Technology including Microsoft and EMR
2 Problem solving
3 Resource providing
