Position Title: Care Manager (RN)
Work Location: Remote-OH (West Central – Dayton, OH)
Assignment Duration: 8 months (Possibility to extend or convert)
Work Schedule: 8a–5p, Monday–Friday
Position Summary:

Implement a 3-way contract with ODM, CMS, and the organization to provide services to vulnerable populations in Ohio so they can remain in their homes.
Background & Context:

Health Plan/Business Unit: Buckeye

Team Culture: 13-person team, highly collaborative with weekly huddles and daily group chats
-
Purpose: Serve members through a federal-state-health plan contract
-
Reason: Backfill for contract CM on leave or resigned
-
Motivator: Continued need for field-based case managers
Key Responsibilities:
-
Required to do field visits (member-facing)
-
Manage caseloads of two groups: over-60 waiver and under-60 population
-
Conduct face-to-face assessments ensuring member safety and welfare

Audit and maintain case documentation
-
Use EMR tasking system to prioritize daily responsibilities
-
Collaborate daily via group chats and weekly huddles
-
Must self-manage, prioritize, and document efficiently
Qualification & Experience:
-
Required Education: Associate’s Degree in Nursing
-
Required Licensure: Registered Nurse (Ohio or Compact RN License)

Experience:
-
Required: Home Health + Behavioral Health + Mental Health + Substance Abuse experience

Disqualifier: Only hospital experience
-
Must be tech-proficient, self-starter, and strong in time management

Must demonstrate waiver knowledge (preferred)
