Title: Senior LTSS Service Care Manager
Location: McAllen/ Edinburg, TX
Duration: 06 months
Shift: M-F 8-5 with possibility of OT when warranted

Job description:
 

• Completing SAI assessments at member homes

• Telephonic outreaches

• Care coordination

• Making and following up on requests for DME (Durable Medical Equipment), therapies, provider services, etc.

• Conducting 28-day follow-ups

• Managing documentation and forms

Performance Expectations/Metrics:

• Adhere to established work processes

• Complete assessments, outreaches, and follow-ups within required turnaround times

First Day Expectations:

• Equipment setup

• Initial training sessions

---

Required Skills/Experience:

• Registered Nurse (RN) with experience in pediatrics

• Proficient with Microsoft Word, Excel, Outlook, and Teams

• Strong communication skills

Education Requirement:

• Associate’s Degree (Required)

• Bachelor’s Degree (Preferred)

Certifications Required:

• RN (Registered Nurse)
