Position Title: Senior LTSS Service Care Manager
Location: Member-Facing - El Paso, TX
Assignment Duration: 6 Months (Potential to Extend/Convert)
Shift: 8 am – 5 pm, may require after-hours visits as per member request
Position Summary:

Evaluates the service needs of the most complex or high-risk/high-acuity members and recommends a plan for the best outcome
Key Responsibilities:

Develops and continuously assesses ongoing long-term care plans/service plans
-
Collaborates with care management team to identify providers, specialists, and/or community resources
-
Coordinates between the member/family/caregivers and care provider team to ensure appropriate person-centered care
-
Monitors care/service plans and member status or progress towards goals
-
Revises care plans as necessary with member/caregivers/providers
-
Monitors member status for complications or clinical symptoms
-
Assesses needs for potential entry into a higher level of care and/or waiver eligibility
-
Reviews member data to identify trends and improve care and compliance
-
Develops care plans based on referrals and intake assessments
-
Collaborates with healthcare providers to revise care plans as needed
-
Documents and maintains all care management activities in compliance with regulations
-
Educates members and families on conditions, care gaps, referrals, and benefits
-
Acts as a liaison and advocate between members, physicians, and facilities/agencies
-
Coordinates community resources and service authorizations (e.g., meals, housing, ADLs)
-
May perform home/site visits (e.g., once a month or more)

Partners with leadership to enhance care quality and cost-effectiveness

May precept clinical new hires and guide through onboarding
-
May support new hires during readiness checkpoints

Collaborates with leaders to monitor onboarding and readiness

Performs other duties as assigned

Qualification & Experience:

Required Education/Certification: ADN or BSN

Required Licensure: RN
