Job Description

Title: Care Manager
Duration: 03 Months - Contract to Hire
Location: 
Member Facing Face-to-face visits 80% work from home /Travel is based on caseload (30%)
Reg 6 -Hardee, Highlands, Hillsborough, Manatee, Polk

Must haves: Pediatric experience of 2 years minimum
 
Nice to haves: Case Management experience


Responsibilities:
  • This person will have metrics of calls that will need to be completed.
  • Phone calls.
  • Coordinate care (finding providers)
  • Assisting in authorization
  • Creating assessments and care plans
  • Ensuring that the children’s needs are met (having issues at school, having a home health aide to assist)
  • Weighted 200 (depends on their acuity)
  • Call volume varies based on need (5-20 calls per day0
  • Contractual obligation is a weighted 200 caseload
  • Tier 3 – 2.2:1
  • Tier 4 – 1:1
Must haves:
2 years of peds experience within case management; must be willing to travel (try to stay within 60 miles radius).
 
Nice to haves:
Behavioral Health, critical and analytical thinker, be able to handle change, more of a social need
 
Candidate Requirements
Requires a Degree from an Accredited School of Nursing or a bachelor’s degree in nursing and 2 – 4 years of related experience. Preferred: N/A  
Required: LCSW/LMHC
NOTE: Evaluates the needs of the member, barriers to accessing the appropriate care, social determinants of health needs, focusing on what the member identifies as priority and recommends and/or facilitates the plan for the best outcome
