Job Title: Care Navigator
Job Location: Remote
Job Duration: 6 Months (Potential to Extend)
Shift: 8 am – 5 pm M-F
9 am – 6 pm Saturdays (alternating – approximately every other weekend)
***includes holidays
 

Responsibilities: Healthy Opportunities (HOP) Eligibility & Enrollments, Benefits Eligibility Screenings, Cold Calls, Assistance in phone queue
Internal/External Groups with which the Candidate will interface:

1.	Proficient with excel/word			1.
2.	Experienced MSW/SW in a healthcare/community setting			2.
3.	Community work, experience with closing SDoH needs			3.
